I went through about 4 pages of forum posts, FAQs, and bans submission info pages, but need more details for submitting a ban. If this have been answered elsewhere and I missed it, I apologize.
The main ban submission page asks for Team and Steamid, which I am guessing the Steamid means my steamid.
The next page asks for name, which I am assuming is my name to tie with my Steamid for submission and not the name of the perpetrator that I am reporting.
I just wanted to verify that before submitting to ensure that it doesn't look like I am trying to say that I am aimbotting.
My next question deals with whether sb_status should only be accessible to game server admins or for all players on our servers. We have many frequent players that I would like to train on the simple requirements for a good demo for Steambans submission, but don't want to make the command publicly usable (if that is possible) if there is reason to keep it restricted to our admins only.

Making the command only available to admins (don't even know if that is possible!) will also limit the possibility to catching cheaters on your servers to admins only. So when a cheater is playing, and there aren't any admins online, it's impossible for others to make use of the steambans community. Why not let them record the (valid!) demo and deal with it. Mordekay, thank you as it really wasn't too obvious to me as I don't know what information you see when you receive the ban submission to know that my information is automatically added. It would be helpful for the page to say something like "Suspected hacker's Steamid:". My reasoning is the entry above it asks for the team that I am submitting for (Team: ), so I am in the train of thought that you need my Team and Steamid. Maybe it is just my bad train of thought, but you did clear it up for me.
BramBo and QuakerOates, my apologies as I asked one of the non-admins in-game (voice) to see if they could use sb_status and they said the response was that they didn't have access to that command or something. Since your replies, I took another of my accounts that does not have admin access on our server and found that the command does work for all. My guess is the player that I asked may have thought that I said "sv_status" or something else. Thank you for clearing that up and reassuring me that there is no security risk with the command being public.
My hat's off to you all. I will get the demo submitted and work on getting our players trained.

 Joined: 29/12/09 Activity:  | You are welcome. However, I was kind of sad when I finished all of the fields to submit a demo (for TF2) to get a pop-up dialog that said "Submissions for game that you selected are currently closed."
Guess I missed a post somewhere. Is there a reference somewhere that lists what games are currently closed for submission and if there is a tentative open date or more details?
At least our community is starting to gear-up on this. We aren't too sure on what to look for in most cases, but after showing my demos on this guy to some of the admins, they have a better idea of what to look for in some cases. I am also getting in the habit of recording demos at least half the time that I play so I am more comfortable and quicker at starting and stopping recordings.
[EDIT: Disregard. It pays to visit the front page of the site from time to time. You already had a notice from a month ago about OB submissions. I will try to pay more attention next time. red = closed, green = open
