The only foolproof method I know that prevents this is for the screen to go black on a players death....
With the new Steam community, ghosting is on the up....

 Joined: 28/12/04 Activity:  | You cannot really prevent ghosting. If some players are not on the same team using teamspeak or any other voice client they can give informations about the other players and you canot do anything about this.
I handle those guys this way: if i have found proof for teamspeak use or i notice that there are some players on the same ip i give them a notice that ghosting will not be tolerated and result in a kick/ban from the server.

We have already enabled the spectating of one team only and we still have the issue.
The problem is that it is nearly impossible to prove it happening.
The only thought I have is that there could be a plugin created that would monitor processes running on the PC. If MSN, XFire, Ventrillo processes were detected you could prevent joining to the server until the process was stopped ie. the application is stopped. If you are in a clan you mostly play with your m8's on a voice server . Just for taking and having fun.
Sure you can abuse the teamspeak but this is not a must.

that is lan ghosting, this thread is about voicechat ghosting
